Terms of Service and Informed Consent Agreement
It is important to know your options so that you can make informed choices. Reading the Frequently Asked Questions on this site will give you many answers. This document gives an overview of your available options, your rights, payment and use of the P2FF Pathway and P2FFTrail Guide support. Please read this form carefully.
Private login access to the P2FF Pathway to Full Freedom and support from a P2FF Trail Guide cannot be provided without indicating that you understand this agreement and accept these terms completely. When your questions have been answered, click on the
"I Agree" button at the bottom of the page. This consent is held private and secure in your personal data base, along with the work you will do on the P2FF Pathway. You may print this statement for your files.
Confidentiality
- Your access, identification and work on this web site are private and held in confidence via your chosen I.D. and password.
- You may view and print your private personal statement, which will show your enrollment status and payment, P2FF Trail Guide hours purchased, used and remaining balance, Steps (P2FF Pathway lessons) paid for, completed or in process, and any purchases from the P2FF Store.
- Use of names will be avoided by P2FF Trail Guide caregivers and we request the same from you. When you communicate in the forums, on the P2FF Pathway, and with your
Trail Guide, please use a generic identifier, such as spouse, friend, child, parent, etc.
- After you have scheduled with your chosen P2FF Trail Guide, you will need to fill out a form asking for a brief history, your goals, your name, phone number, address, age and gender so that your Trail Guide can give you appropriate care.
- P2FFTrail Guide caregivers participate in regular supervision and continuing education sessions where current cases are reviewed and receive prayer coverage. Again, your privacy will by honored and names and specific identifiers will be avoided.
- If your needs require a referral to another service, you will be given options from our
P2FF Authorized Referral List.
- If you will not or cannot receive the help you need and it is determined that you (the P2FF Pathfinder) are dangerous to yourself or to others, you will be informed and the necessary report will be made to proper authorities.
Payment and Utilization Policy
P2FF STORE:
- A very brief Customer Satisfaction Response will be emailed to you shortly after goods have been sent.
- Items sold have appropriate tax, shipping and handling fees added on to the purchase price.
- Wearable items can be returned for credit when returned in “new” condition.
- Questions and concerns can be directed to 303-646-2041 ext 1.
- No refunds are given but credit can be considered.
P2FF PATHWAY:
- Enrollment for private login access to P2FF PATHFINDERS and utilization of Steps (lessons) and practical “helps” and tools on the P2FF Pathway is paid for by the year or by multiple year packages.
- A very brief Customer Satisfaction Response will be emailed to you after you complete each Step of the Pathway. You are encouraged to complete and submit an evaluation of the Steps at any time.
- Credits may be considered for services but no refunds will be given for missed or unused services.
- If paid fees are not used within six (6) months, we will notify you by email and encourage you to use your paid services and continue your P2FF Pathway.
- When your enrollment expires or when there has been no activity within the twelve (12) months after payment, Step (lesson) and P2FF Trail Guide fees will be forfeit. Use your paid services at least monthly to maintain continuity of care and at least once per year to maintain credit and up-date your contact information.
Though you may work through the P2FF Pathway to Full Freedom by yourself, it is most effective when partnering with a healthy, mature friend or counselor. Another excellent option is to receive the support of a caring, trained and knowledgeable P2FF Trail Guide.
It is in the best interest of the care receiver and caregiver to understand options in care and what is expected from each participant. P2FF Trail Guide caregivers are lay people, not acting as medical or counseling professionals. P2FF strictly prohibits your Trail Guide caregiver from providing any medical services to you. For emergency or professional medical or counseling services, contact an emergency room or consult with a properly licensed caregiver. Pathway to Full Freedom. Trail Guides provide prayer and personal support, encouragement and accountability as your Creator God heals and frees you from recognized and unrecognized wounds and lies. When your needs are beyond the services of your P2FF Trail Guide, you will be requested to seek appropriate professional services in your area. P2FF Trail Guides can provide information about licensed professionals to you from our Approved Referral List. Professionals on our Approved Referral List are independent of Pathway to Full Freedom and are completely responsible for their services apart from
P2FF , its agents and associates. P2FF Trail Guide caregivers are in
P2FF training and supervision specific to this lay ministry. If your caregiver is a medical professional or counselor, he or she may make arrangements to serve you according to law under their own licensure and in their specific professional arena outside of Pathway to Full Freedom responsibility and involvement. It is important that you inform your P2FF Trail Guide of all professional and other care you are receiving.
P2FF Trail Guide SUPPORT:
- P2FF Trail Guide support will be available with Enrollment and private access into the P2FF Pathway to
Full Freedom. Fees for support from your chosen Trail Guide will be charged and paid for by VISA, MC, or Discover credit cards in one-hour increments. Your payment will be held in your account until you use the paid hours. After your support session, the fee amount is deducted and your P2FF Trail Guide caregiver is paid.
- P2FF Trail Guides provide care via phone. With written consent of both parties, care can include other forms of communication.
- A very brief Customer Satisfaction Response will be emailed to you after your 1 st, 7 th and last session. You are welcome to complete an evaluation of services, available on the Pathway, at any time.
- The paid one-hour units can be used in 30 minute, 60 minute or 90 minute sessions, as arranged with your P2FF Trail Guide caregiver.
- Paid care sessions can be cancelled at no loss with at least 24 hours prior notice. If an appointment is missed without 24 hours prior notice, the fee for that session is forfeit, the appropriate amount will be deducted from your account, you will be notified by email, and your P2FF Trail Guide will be paid for the time held for you.
- The P2FF Trail Guide caregiver hourly fee varies according to the education, training and experience of each P2FF Trail Guide. You, the P2FF Pathfinder, have the right to request care from available P2FF Trail Guides and discontinue care at any time for any reason. Each P2FF Trail Guide caregiver also has the right to decline payment and the caring relationship for any reason. This could happen for a variety of scheduling, personal or other reasons. We will make every effort to prepare you for necessary changes and make those changes with as little interruption of your support as possible.
- You may transfer your paid hours between available Trail Guide caregivers after filling out the Request for Trail Guide Change. Please understand that maintaining a relationship with one P2FF Trail Guide through your healing journey provides the best continuity and lasting results.
- After making your choice of the available P2FF Trail Guides, your request will be forwarded via email to that person with your desired contact times. The Trail Guide will then confirm your desired schedule or propose another schedule. If an appointment cannot be agreed upon, you may need to wait or choose another P2FF Trail Guide.
- For your session, when the schedule has been finalized, you will call the web site phone number and the extension for your P2FF Trail Guide at the pre-arranged time to receive your care session. At least 10 minutes before your session is to end, your Trail Guide will remind you of the remaining time so that you can tie up loose ends and prepare for your next session.
P2FF BODY AWARENESS and PROCESSING:
- Face-to-face physical therapy, P2FF Body Awareness, processing and training services are scheduled, charged and handled separately, through the “Pathfinder
login” (Home Page) part of this web site.
- Insurance may or may not be billed, but you can print your receipt for payment off of this web site to use lawfully. P2FF is non-profit organization.
Most receipts are tax-deductible, but ask your tax professional.
You have the right to ask about your care in all areas by emailing to the appropriate department in Contact Us. It helps us make improvements if you complete Customer Satisfaction Responses and evaluations for us.
